Crossroads has moved our office.
We are unable to allow our patients email access to either the providers or other staff in our group. Please call our office with any questions or medical issues you may have.
No Show Policy:
We do charge for missed appointments and for late cancellations. This ensures that our schedule stays full and that we are able to get patients in our office in a timely manner. If you have extenuating circumstances, that will be taken into consideration by your provider.
We require 24 hour notice of any cancellations. This means that Monday appointments need to be cancelled by Friday the week before. New patient missed appointment fees are $100.00. Existing therapy patients will be charged $75.00 for first occurrence and $100.00 for subsequent occurrences. For Drs. Cottle and Jennings and Clay Shugart and Teresa Hurst, the fee for existing patients is $50.00 per occurrence. (Missed appointments include less than 24 hour notice of a cancellation.)
If you feel as though your condition is life threatening, contact 911 immediately for transport to the nearest hospital or emergency clinic.
Medication Refill Policy:
We understand that taking your medication regularly is important to your care. If you need a refill, please contact your pharmacy in plenty of time ahead, so that there are no delays in your medication regimen. Two to three days ahead is preferred. We fax back prescriptions and have nurses call in prescriptions after 5pm each day. If you have a controlled substance refill that needs to be written, please contact us at least three days ahead of time and let us know. We will arrange for the pick up of your written prescription. There is no guarantee that you will receive a refill same day if you contact us with less than 24 hour notice.
